If you are a new client, you can access and print applicable forms. Please select “adult clients” or “child clients” below, as applicable, open the forms, fill them out on your computer, print them, and bring them with you to your first appointment.

When you come to your first appointment, please be sure you have a seat in our waiting area. Dr. Kugler will come out to greet you when it is time for your appointment. There is no need to let her know you are waiting outside the office.
